The prevalence of type 2 diabetes has been increasing exponentially. Patients with diabetic neuropathy from type 2 diabetes report a significantly decreased quality of life caused by painful diabetic neuropathy (PDN). PDN causes a length-dependent burning, tingling, or “shock-like sensations” with increased sensitivity to both painful (hyperalgesia) and nonpainful stimuli (allodynia). These symptoms frequently persist despite the use of currently available treatments. The development of mechanism-specific treatments for PDN is in an urgent need. We use db/db mice as a model of PDN to study the molecular mechanisms underlying PDN. Accumulating evidence suggests that nerve growth factor (NGF)-mediated neuronal signaling is essential for the development of PDN. The NGF-p38 signaling in primary sensory neurons induces cytokine upregulation and neurogenic inflammatory reactions in peripheral nerves. Inhibition of NGF-p38 signaling significantly reduces cytokine-mediated inflammation and pain behaviors. Our results provide evidence that new treatments that target NGF-mediated neurogenic inflammation could be effective for treating PDN from type 2 diabetes. |
